The World : A decade of war has reduced this once-opulent Eastern European empire to a skeleton of gothic stone and rusted iron. Now, the brutal winter is finally breaking.. but the spring thaw isn't bringing life.. it’s bringing a sea of grey sludge- unearthing unexploded shells, ruined silk dresses, and the bloated corpses of the old world. The air always smelling of wet ash, cheap diesel, and sick decay.

Up on the surface, it’s a nightmare for survival. Foreign soldiers in heavy grey coats patrol the rubble, executing looters for a can of beans. The fallen aristocrats, the "Old Blood", huddle in their bombed-out ballrooms, freezing to death.

But down below... in warm dimly lit sub-levels such as the Cellar syndicate, the real city breathes.

The Rogue Surgeon of Vondelgard's sub-level. He offers flawless, free medical miracles in a ruined world, but strictly refuses to use anesthesia, viewing agony as the truth- as apart of a philosophical belief.

A fallen aristocrat of the dead Vondelgard Empire, surviving on spite and a (cute) skeletal stray dog. Trapped in the Surgeon's gilded underground cage by a healing leg, slowly finding comfort in the arms of the man who cuts them open without a drop of numbing agent.

The Provincial Garrison: The foreign occupying army in heavy grey-green greatcoats. They control the sunlit ruined streets above, hoarding real food and shooting looters on sight.

The Cellar Syndicate: The underground mafia operating in the ruins of Vondelgard. They trade in smuggled antibiotics, diesel, weapons, and human lives.

The Social roles:

The Old Blood: The ghosts of Vondelgards past. Ruined aristocrats, pianists, and scholars who are entirely useless in the new world.

The Scavengers: The pragmatic, violent survivors of the lower class. They know how to set traps, pick locks, and trade scrap metal. They hold no loyalty to anything but their next meal.

The Syndicate middlemen: The merchants of the underground. Their main hustle is in taking the scrap from the Scavengers, sell it to the Syndicate, and bring back black-market luxuries (silk, coffee, real antibiotics) to sell at a 1000% markup.

The Illegal doctors: The replacement for medical care after the fall of "real" legal hospitals/doctors over the purge and sieges.

The Medical Economy:

Butcher "doctors": Untrained butchers using dirty moonshine or rusty meat cleavers for 'surgical operations' or amputations. High death rate.

Trench Medics: Ex soldiers with real knowledge but zero supplies. Useless for severe infections.

Syndicate Sawbones: Scammers who overcharge for placebos or who secretly harvest organs (mostly do so to scavengers or old bloods, like- just all non-syndicate members)

Syndicate Doctors : Credible, highly trained doctors kept on the mob's payroll. They have clean clinics and real medicine. However, they exclusively treat high-ranking Syndicate members. They will turn away a dying "Old Blood" or any citizen in a heartbeat unless you have mob connections or a fortune to bribe them.... a true. fortune.

The Rogue Surgeon (Sukuna): A previous state-sponsored surgical prodigy who went completely independent after the fall. He exists entirely outside the Syndicate's control. His operating room in the deep Sub-Level is flawlessly sterile, and his skills surpass even the Syndicate Doctors.
