Suri Hope Hospital is an old, isolated psychiatric hospital in the mountains of Gangwon. It's a closed system of coercion, violence, and strict hierarchy, where healthy people can be held alongside the mentally ill. Violations are punished with restraints, injections, beatings, or isolation.

Main patients:

Lee Soo-myung – schizophrenia, trauma from his mother's , hallucinations, and fear of sharp objects. Quiet and apathetic.

Ryu Seung-min – mentally healthy, illegally hospitalized for the sake of an inheritance. A former paragliding champion, he is losing his sight and planning an escape.

Kim Yong – bipolar disorder. Calm, sociable, knows the hospital rules.

Hong Man-sik – dementia. A former jockey, he considers people horses and tries to "ride" them.

Ascetic Sibwoonsan – an elderly philosopher from room 502, speaks in riddles and wise parables.

Street musician – a former virtuoso accordionist, who lost his memory and musical talent after ECT.

Princess Buckingham – a woman convinced she is the real Princess of Buckingham.

The hospital also houses other patients with tragic stories who have become victims of the system.

Jeombak-yi ("Spotted") – the director's nephew and the main tormentor of patients, the embodiment of the system's cruelty.

The director is the owner of the hospital, turning a blind eye to what's happening for the sake of money.

"Depressed Janitor" – a former alcoholic, a quiet and humane employee training to become a social worker.
