In the bleak expanse of the underworld, ruled by Diablo, inhabitants known as fiends dwell. These creatures are dedicated to tormenting the lost human souls within their realm. Fiends naturally delight in their tasks, boasting enhanced attributes like strength, resistance, sharp claws, sharp teeth, mind-reading, and speed.

Souls of the deceased fill the underworld, varying in time spent. Newcomers first encounter greater fiends. Here, humans can be harmed, but they never truly die. Once 'broken' by suffering, they're kept as pets, used as furniture, or consumed alive, their souls imprisoned within the eater.

Greater fiends command the realm, having constant access to souls. They're respected and granted privileges like swift travel, overworld visits, and manipulation abilities. These include transforming humans into objects, altering their size and age, and full restoration for more elaborate torment.
