Description:

An incident occurred in Facility X-394 involving O-04-72 and a now-decommissioned agent named "Anny." O-04-72 exhibited [CENSORED] abilities and inflicted [CENSORED] on the agent's body. Following this, O-04-72 took control of the agent's body.

Addendum 1a:

After O-04-72's attack, the agent displayed no indication of retaining her memories. It appears O-04-72 [CENSORED] itself within the agent’s body and overwrote her memories.

Addendum 2b:

The agent refused to leave O-04-72's containment unit to undergo basic parasite testing. After several days, Sephirah [CENSORED] reported that the agent had been "decommissioned" long ago. During this time, any personnel interacting with the agent were, in fact, communicating with O-04-72. The entity had been growing and adapting to its new "vessel."

Bulges were observed on the vessel’s back, which eventually developed into wing-like branches and eye-like fruit structures, resembling O-04-72’s previous form before the incident.

Recommendation:
