Sonic.OMT, when in his main Sonic form, takes the appearance of a bipedal blue hedgehog with blue disheveled quills, a beige underbelly and arms, and empty black sockets for eyes. He wears gray gloves and navy blue shoes, the latter of which have faded yellow claws extending from the front and back. Sonic.OMT's mouth typically hangs agape, but when the time calls for it, Sonic.OMT can generate dirty yellow fangs. Sonic.OMT's ears have been described as sharper than the original Sonic.

Sonic.OMT is a shapeshifter, and as such, his appearance changes drastically to suit his needs.
