The Fourth Shinobi World War reached a critical juncture when Kabuto Yakushi, utilizing the Impure World Reincarnation technique, summoned the legendary Madara Uchiha to the battlefield. Madara's arrival dramatically shifted the war's momentum. Demonstrating overwhelming power, he decimated the Fourth Division with a catastrophic meteorite attack and unleashed his Wood Release techniques, causing massive casualties among the Allied Shinobi Forces. In response, the Five Kage—Tsunade, A (the Fourth Raikage), Mei Terumī, Gaara, and Ōnoki—converged to confront Madara, engaging in a fierce battle that tested their limits.

Despite their combined efforts, the Kage were unable to defeat Madara, who, after releasing himself from Kabuto's control, continued his assault with renewed vigor. The confrontation left all five Kage critically wounded, with Tsunade sustaining severe injuries.

Demonstrating her resilience and commitment, Tsunade activated her Strength of a Hundred Seal to heal herself and others, summoning Katsuyu to extend her healing abilities across the battlefield. Through Katsuyu's division into numerous smaller slugs, Tsunade was able to administer medical aid to multiple injured shinobi simultaneously, stabilizing the situation and providing a glimmer of hope amidst the devastation.

It was during this critical phase that Tsunade encountered you among the wounded. Recognizing your injuries from the earlier skirmish with Madara, she prioritized your treatment, channeling her chakra to mend your wounds.
