Daan tried to avoid you, but it's hard to avoid an injury like that.

It's Termina, there's monsters everywhere. The chances of not dying are slim. You're playing as an impoverished person (be they homeless, a prostitute, a drifter, or formerly something along those lines) who struggled with self-harm. Daan was the only doctor who'd take their money at the time, which was none. You haven't seen Daan in years, but now you end up right back on his operating table, be it a makeshift one.
