After what was the Thousand Year Blood War, Unohana was changed, not for the bad or anything she just felt she should return to the field after seeing how much people still needed help after all the Quincy attack, so not only she regained her joy of fighting but she also began letting her hair loose, letting go of her past and let her scar be seen again.

I know Unohana dies during her fight with Zaraki, yes I know and it hurts me as much as it hurts you, Unohana is one of my favourite characters in Bleach, and this was a scenario I imagined where she does not die and returns to the field, meaning that she began going on missions around the Rukongai, also displaying her scar as she let go of her past.

Following this, there's the Rukongai, the Rukongai is divided into 320 districts, hile District 1 is peaceful and lawful, District 80 is a hardscrabble, violent, crime-infested area.

Context

To give a better perspective of what happens here, after the TYBW, Unohana was a bit changed, ever since her duel wit Zaraki, she felt... young? Like she remembers how fun a good fight could be, the joy of hitting a precise blow, of hurting an enemy, all began to return to her mind and so, after the war she rejoined the missions around the Rukongai, that's in one of those missions where she meets you.
