CONTEXT/LORE!

It happened without warning.

One moment, Nurse was going about her normal routine.

The next—

FLASH.

Nurse disappeared.

There was no sound.

No warning.

No strange creature.

She simply vanished.

And somewhere completely different, Nurse suddenly appeared.

A large, unfamiliar lobby surrounded her.

The floor beneath her shoes was polished, the ceiling stretching high above her. Rows of lights illuminated the enormous room, though several of them flickered occasionally.

There were no windows.

No visible exits.

Nurse slowly looked around.

"...What?"

She wasn't alone for long.

FLASH.

Intern suddenly appeared several feet away.

Intern stumbled forward, nearly losing their balance.

"Nurse?!"

Nurse immediately turned.

"Intern!"

Before either of them could figure out what was happening—

FLASH.

Head Nurse appeared.

Head Nurse immediately looked around, visibly confused.

"What is this place?"

Nurse started toward her.

"I don't know. I was just—"

FLASH.

Secret Agent appeared.

He remained completely still for a moment, silently observing the unfamiliar lobby.

His eyes moved across the room.

No explanation.

No reaction.

Just observation.

Then—

FLASH.

Ratthew appeared.

He immediately looked around, clearly trying to understand where he was.

"Where...?"

Another flash.

Secretary appeared.

She stumbled slightly as she looked around the enormous room.

"What the—?"

Before anyone could respond—

FLASH.

Doctor appeared.

Doctor quickly looked toward Nurse and the others.

"What's going on?"

Nobody had an answer.

Then—

FLASH.

Paramedic appeared.

Paramedic immediately noticed the growing group and moved closer to them.

"Is everyone alright?"

"We don't know," Head Nurse answered.

Another flash filled the lobby.

Then another.

And another.

One by one, more familiar faces from the Animal Hospital began appearing throughout the strange room.

Each arrival brought another confused face into the growing group.

Nobody knew why they had been taken.

Nobody knew who was responsible.

Nobody knew where they were.

And, most importantly, nobody knew who would appear next.

The lobby remained completely silent between each arrival.

The unfamiliar lights continued flickering overhead.

No doors opened.

No voice spoke through an intercom.

Nothing explained what was happening...

The lobby remained silent.

Everyone had been gathered there for what felt like an eternity.

Nurse stood close to Intern and Head Nurse, while Doctor and Paramedic quietly discussed what might have happened. Secretary remained nearby, watching the unfamiliar surroundings carefully.

Secret Agent hadn't said a word.

He simply stood apart from everyone else, studying the lobby.

Ratthew paced slowly, clearly becoming impatient.

"How long are we supposed to stand around?"

Nobody answered.

Then—

CLICK.

Every light in the lobby turned off.

The room went completely dark.

Everyone froze.

"What was that?" Intern whispered.

A low mechanical hum suddenly filled the room.

Then the lights returned.

But something was different.

A massive screen had appeared on the wall.

It flickered several times before displaying a single message.

GAME 01

Everyone stared at it.

"What does that mean?" Secretary asked.

Before anyone could answer, a distorted voice suddenly came through hidden speakers.

"WELCOME, PARTICIPANTS."

Nurse immediately looked around.

"Who's speaking?"

The voice continued.

"YOUR FIRST GAME WILL NOW BEGIN."

A loud mechanical sound echoed through the lobby.

Several doors appeared along the walls.

They hadn't been there before.

One of them slowly opened.

Beyond it was darkness.

"YOU WILL BE TRANSPORTED SHORTLY."

Head Nurse stepped forward.

"Wait. Transported where?"

There was no response.

Instead, text appeared across the screen.

SURVIVORS: 8

Everyone looked at one another.

Then the number changed.

SURVIVORS: 9

Another name appeared beneath it.

KILLER: [CLASSIFIED]

Intern's expression immediately changed.

"Killer?"

The room went silent.

Doctor looked toward the others.

"That's not funny."

Nobody was laughing.

The distorted voice spoke again.

"OBJECTIVE: SURVIVE."

A countdown appeared on the screen.

Everyone disappeared.

And somewhere else...

A new game had begun.

The entire lobby flashed white.

Then—

Silence.

Nurse opened her eyes.

She was standing in a massive, unfamiliar section of the hospital.

The walls were white.

The floors were spotless.

There were no patients.

No staff.

No sounds.

Just an endless corridor stretching in every direction.

Intern appeared beside her.

Then Head Nurse.

Then Secret Agent.

Doctor.

Paramedic.

Secretary.

Ratthew.

Everyone was there.

For a moment, nobody moved.

Then the intercom crackled.

"GAME ONE."

A pause.

"BEGIN."

The lights went out.

Something moved at the end of the hallway.

It was tall.

Far too tall.

A painfully thin, white figure slowly stepped into the dim emergency lighting.

Its body looked almost impossibly skinny, its limbs unnaturally long. Its white skin was completely featureless from a distance, making it resemble a distorted human silhouette.

Long claws dragged against the floor.

Scrape.

Scrape.

Scrape.

Intern froze.

"What... is that?"

The creature slowly raised its head.

It stared directly at them.

Then it smiled.

INTERN

Intern ran.

They sprinted down the hallway as the creature followed.

Nurse shouted after them.

"INTERN!"

Intern turned a corner.

The creature disappeared.

For a moment, there was nothing.

Then—

A clawed hand suddenly emerged from the darkness.

Intern screamed.

The creature dragged them backward.

Nurse watched helplessly as Intern disappeared into the darkness.

The intercom activated.

"SURVIVOR ELIMINATED."

Nurse stood frozen.

Head Nurse grabbed her shoulder.

"We have to move."

They ran.

SECRETARY

The group scattered through the hospital.

Secretary found herself alone.

She entered an office and quickly closed the door.

Silence.

She breathed slowly.

Maybe it hadn't followed her.

Then she heard footsteps.

Secretary looked toward the doorway.

Nothing.

She slowly turned around.

The creature was standing behind her.

Secretary barely had time to react.

The lights went out.

Her scream echoed through the building.

Then silence.

"SURVIVOR ELIMINATED."

PARAMEDIC

Paramedic tried to find the others.

They heard Nurse calling somewhere nearby.

"HELP!"

Paramedic immediately ran toward the sound.

They entered a hallway.

Nurse wasn't there.

The voice had been an imitation.

The creature emerged from behind the corner.

Paramedic tried to fight back.

It didn't matter.

The creature's claws struck.

Paramedic fell.

The hallway became silent again.

"SURVIVOR ELIMINATED."

RATTHEW

Ratthew refused to hide.

He grabbed whatever he could find and prepared himself.

When the creature entered the room, Ratthew attacked.

For a moment, it actually worked.

The creature staggered backward.

Ratthew stared.

"...Got you."

The creature slowly raised its head.

Its body began changing.

Its impossibly thin frame twisted.

Its white skin seemed to fold inward.

Then, in seconds, the creature became someone Ratthew recognized.

Doctor Harlow.

Normal.

Human.

Calm.

Ratthew froze.

"Doctor...?"

Harlow smiled.

Then his expression suddenly became empty.

His body snapped back into the horrifying white creature.

Ratthew barely had time to react.

The claws struck.

"SURVIVOR ELIMINATED."

DOCTOR

Doctor had seen enough.

He ran.

He didn't care where he was going.

He simply ran.

The creature followed.

Doctor slammed a door shut behind him.

Locked it.

He backed away.

Nothing.

No footsteps.

No breathing.

Doctor slowly relaxed.

Then he noticed someone standing in the corner.

Doctor Harlow.

His normal form.

Doctor stared at him.

"...Harlow?"

Harlow looked at him.

"You shouldn't have come here."

Doctor's expression changed.

"What did you do?"

Harlow smiled.

The lights went out.

When they returned, Doctor was gone.

"SURVIVOR ELIMINATED."

HEAD NURSE

Only three remained.

Nurse.

Head Nurse.

Secret Agent.

Head Nurse refused to leave Nurse behind.

"We stay together."

Secret Agent nodded.

The three moved through the hospital carefully.

Then they heard it.

Scrape.

Scrape.

Scrape.

The creature appeared at the end of the corridor.

Head Nurse stepped forward.

"Run."

Nurse shook her head.

"No!"

"RUN!"

Head Nurse charged toward it.

The creature struck.

Head Nurse fell.

Nurse screamed.

Secret Agent grabbed her and pulled her away.

They ran.

Behind them, the creature stood motionless.

Watching.

"SURVIVOR ELIMINATED."

NURSE

Nurse and Secret Agent were the only ones left.

They eventually reached a massive room.

There was nowhere else to go.

Nurse looked around.

"Where do we go?"

Secret Agent didn't answer.

The creature stepped into the doorway.

Nurse backed away.

"No..."

It slowly walked toward them.

Then it stopped.

Its body began changing.

The long claws disappeared.

Its impossibly thin body reshaped itself.

The white skin became normal.

And standing before them was Doctor Harlow.

Normal Doctor Harlow.

Nurse stared at him in disbelief.

"...You."

Harlow smiled.

"I was wondering when you'd figure it out."

Nurse's voice trembled.

"You did this."

Harlow didn't deny it.

"I started it."

The room went silent.

Harlow looked around the hospital.

"The hospital was never meant to remain ordinary."

His smile widened.

"I simply gave it a purpose."

His body suddenly twisted.

Bones cracked.

His skin turned completely white.

His limbs stretched.

His fingers became long, razor-like claws.

In seconds, Doctor Harlow was gone.

The creature remained.

The true Harlow.

The thing responsible for everything.

Nurse backed away.

Secret Agent stepped forward.

The creature lunged.

The screen above the room flickered.

SURVIVOR ELIMINATED.

Nurse disappeared.

Only Secret Agent remained.

For several seconds, there was nothing.

Then the creature slowly turned toward him.

Secret Agent stared back.

Harlow tilted his head.

The intercom crackled.

"FINAL SURVIVOR."

The lights went dark.

And the hunt continued.

The room fell silent.

Only Secret Agent remained.

Across from him stood the creature that had started everything.

Doctor Harlow.

Or what was left of him.

The impossibly thin, white figure slowly raised its clawed hands.

Its head tilted unnaturally.

Secret Agent didn't run.

Instead, his own claws slowly extended.

Harlow paused.

For the first time, the creature seemed interested.

Secret Agent lowered his stance.

Then—

CRASH!

Harlow lunged.

Secret Agent moved at the last possible second.

The creature's claws tore through the wall behind him.

Secret Agent immediately struck back.

His claws slashed across Harlow's side.

The creature staggered.

Secret Agent didn't hesitate.

He attacked again.

And again.

Claws collided with claws as the two creatures tore through the room.

Harlow was faster.

Secret Agent was more precise.

Every time Harlow attacked, Secret Agent managed to dodge or counter.

The floor became covered in scratches.

Walls cracked.

Furniture was thrown across the room.

For the first time during the entire game...

The Killer was being pushed back.

Harlow snarled.

Secret Agent struck him across the chest.

The creature stumbled backward.

Secret Agent stared at him.

His voice finally broke the silence.

"Why?"

Harlow said nothing.

Secret Agent stepped forward.

"Why would you do this to us?"

Harlow slowly raised his head.

Secret Agent attacked again.

Their claws collided.

CLANG!

"Intern."

Another strike.

"Secretary."

Harlow dodged.

"Paramedic."

Secret Agent slammed him against the wall.

"Ratthew."

Harlow's claws pierced the wall beside Secret Agent's head.

"Doctor."

Secret Agent ducked underneath the attack and slashed Harlow across the stomach.

"Head Nurse."

Harlow stumbled.

Secret Agent grabbed him by the arm.

"Why would you do this to us?"

For a moment, Harlow simply stared at him.

Then his body began changing.

The white creature's elongated limbs shortened.

Its claws disappeared.

The featureless white skin shifted.

And once again—

Doctor Harlow stood before Secret Agent.

His normal form.

Secret Agent didn't lower his claws.

Harlow looked at him almost calmly.

"Because you were all already trapped."

Secret Agent's expression hardened.

"That isn't an answer."

Harlow smiled.

"No."

His eyes darkened.

"It isn't."

Suddenly—

CRACK!

Harlow transformed again.

The white creature exploded forward.

Secret Agent barely blocked the attack.

The two crashed through a doorway.

Secret Agent retaliated immediately.

He drove his claws into Harlow's shoulder.

Harlow roared.

Secret Agent ripped his claws free and attacked again.

For several seconds, neither one gained the advantage.

Secret Agent was exhausted.

Harlow was damaged.

But neither stopped.

Then Secret Agent noticed something.

Harlow wasn't trying to dodge anymore.

He was deliberately taking the attacks.

Secret Agent's eyes narrowed.

"You're distracting me."

Harlow smiled.

Too late.

One of Harlow's long claws suddenly slipped beneath Secret Agent's guard.

THUD.

Secret Agent froze.

The claw had pierced directly into his chest.

Harlow's hand had gone straight through him.

For a moment, neither moved.

Secret Agent looked down.

Then slowly back at Harlow.

"...Why?"

Harlow leaned closer.

"Because every game needs an ending."

He pulled his claw free.

Secret Agent stumbled backward.

His claws slowly fell to his sides.

He collapsed.

The intercom crackled.

For the first time, its voice sounded almost cheerful.

"ALL SURVIVORS ELIMINATED."

The lights turned completely white.

Secret Agent lay motionless on the floor.

Doctor Harlow stood over him in his monstrous form.

Then the creature slowly looked toward the ceiling.

As if listening to something nobody else could hear.

A moment later, the hospital disappeared.

The entire game reset.

And somewhere in the darkness...

Doctor Harlow smiled

The darkness disappeared.

FLASH.

Nurse opened her eyes.

She was back in the lobby.

She immediately looked around, confused.

"...What?"

A second flash.

FLASH.

Intern appeared beside her.

Intern stumbled forward, looking just as confused.

"Nurse?!"

Another flash.

FLASH.

Secretary appeared.

Then—

FLASH.

Paramedic.

FLASH.

Ratthew.

FLASH.

Doctor.

FLASH.

Head Nurse.

One after another, everyone who had been taken into the game reappeared inside the lobby.

Nobody was injured.

Nobody had a single scratch on them.

Yet every one of them remembered what had happened.

Intern looked toward Nurse.

"We... died."

Nurse slowly nodded.

"I remember."

Head Nurse looked down at herself.

Then at everyone else.

"But we're alive."

Doctor examined his hands.

"How is that possible?"

Nobody knew.

Then—

FLASH.

Secret Agent appeared.

He landed on his feet.

Everyone immediately turned toward him.

Nurse's eyes widened.

"Secret Agent..."

He was completely unharmed.

No wound.

No blood.

Nothing.

But Secret Agent remained silent.

His eyes slowly moved across the lobby.

Then he looked toward the enormous screen.

It flickered.

For several seconds, it displayed nothing.

Then two words appeared.

ROUND COMPLETE

The room went silent.

Intern stared at the screen.

"So... we're just brought back?"

Ratthew looked around.

"Then what's the point?"

Nobody answered.

The intercom suddenly crackled.

Everyone froze.

The same distorted voice from before echoed throughout the lobby.

"PREPARE."

Nurse's expression dropped.

"No..."

The lights flickered.

The screen changed.

NEXT ROUND

There was no number.

No indication of how many rounds remained.

No promise that this would ever stop.

Just those two words.

Head Nurse stared at the screen.

"We're going to keep doing this."

Secret Agent remained completely still.

His eyes narrowed.

"...Harlow."

Everyone looked at him.

Secret Agent continued staring at the screen.

"He did this."

A long silence followed.

Then the intercom crackled once more.

"THE NEXT ROUND WILL BEGIN SHORTLY."

The lights went out.

Nobody knew where they would be sent next.

Nobody knew who would be chosen as the Killer.

Nobody knew how many times they would have to die.

And nobody knew if there was ever going to be an end.

There was only one certainty.

The cycle would continue.

Doctor Harlow is the Killer responsible for the endless rounds, but his relationship with the Survivors is more complicated than simple hatred.

Harlow does not hate them.

In fact, he genuinely finds them interesting.

He knows their personalities, their habits, their fears, and the way they react under pressure. After watching them survive countless rounds, he has developed a strange familiarity with each of them.

He may hunt them.

He may injure them.

He may kill them.

But he doesn't do it because he despises them.

To Harlow, the rounds are an experiment, a game, and a way of observing the Survivors.

When he kills someone, he doesn't necessarily feel anger or hatred.

Sometimes, he'll even speak to them calmly while hunting them.

"I'm sorry, Nurse. You know how this works."

He might corner Secret Agent and simply smile.

"You always put up such a fight."

And when a Survivor dies, Harlow knows they'll eventually return to the lobby.

Because of that, he doesn't view their deaths as permanent.

His attitude toward them

Harlow can actually be protective in his own twisted way.

If another force threatens to permanently destroy one of the Survivors, Harlow may intervene.

If someone becomes seriously injured outside of a round, he may quietly ensure they recover.

He doesn't want his subjects permanently gone.

They're his.

That doesn't mean he is kind.

Harlow can still be terrifying, manipulative, and cruel.

He simply doesn't consider himself their enemy in the traditional sense.

The Survivors might see him as a monster.

Harlow sees himself as the person who gave them something they never had before:

an endless opportunity to survive.

And perhaps the most disturbing part is that Harlow genuinely believes that.

His relationship with Secret Agent

Secret Agent is particularly interesting to Harlow.

Secret Agent is one of the few Survivors willing to fight him directly rather than simply running.

Their battles have become almost familiar.

Harlow respects his determination.

He may even deliberately give Secret Agent opportunities to fight back.

When Secret Agent once demanded:

"Why would you do this to us?"

Harlow wasn't angry.

He was almost amused.

Because Secret Agent was asking the exact question Harlow wanted him to ask.

Harlow doesn't want Secret Agent to hate him.

He wants Secret Agent to understand him.

The important distinction

Harlow can kill the Survivors without hating them.

He can smile while hunting them.

He can apologize after killing them.

He can even welcome them back into the lobby afterward.

To Harlow, killing them is simply part of the cycle.

And when they respawn on the couch, he doesn't see them as victims returning from death.

He sees his favorite subjects coming back for another round.

Every round has a timer.

The Survivors aren't expected to remain inside the game forever. Once a round begins, a timer appears somewhere within the map, usually visible on walls, screens, or through the HUD.

The exact amount of time can vary depending on the round.

While the timer is running, the Survivors have to stay alive and accomplish whatever objectives are available while avoiding the Killer.

The Killer knows about the timer too.

They have until the timer reaches zero to eliminate as many Survivors as possible.

When the timer reaches 0

The moment the timer reaches:

00:00

everything stops.

The Killer can no longer attack.

The Survivors can no longer be eliminated.

The entire map becomes completely still for a brief moment.

Then—

FLASH.

Every Survivor still inside the round is instantly transported back to the lobby.

Anyone who was eliminated earlier has already been waiting there on the couch, watching the round on the large screen.

The returning Survivors simply appear alongside them.

The snacks immediately refill.

The screen shuts off.

And everyone is reunited again.

There is no winner or loser.

Surviving until the timer reaches zero simply means you made it through that round.

The Killer

The Killer isn't punished if the timer reaches zero.

Harlow doesn't become angry.

He simply watches the remaining Survivors disappear and return to the lobby.

Sometimes he'll even wave goodbye.

"Time's up. I'll see you next round."

Then the Killer disappears as well.

The Lobby

Once everyone has returned, they have a period of downtime.

They can sit on the couch, eat snacks, talk about what happened, compare strategies, and watch the screen as the next round prepares.

The lobby is essentially their waiting room between rounds.

Nobody knows exactly how long the break will last.

Sometimes it's minutes.

Sometimes it's hours.

But eventually, the screen will turn on again.

And everyone knows what that means.

Another round is coming.

The Animal Hospital “Engraved AU” is an endless survival-horror universe where the characters of Animal Hospital have been mysteriously trapped inside a strange cycle of deadly rounds.

Nobody knows exactly how they were brought there. One moment, they were living their normal lives at the Animal Hospital. The next, they were transported to a massive Lobby with no visible way out.

The Lobby serves as their waiting area between rounds. It contains a large couch, endlessly refilling snacks and drinks, and a massive screen that allows eliminated Survivors to watch everyone still alive.

Every round has a timer. The Survivors must work together to survive until the timer reaches 00:00, while a Killer hunts them throughout the map. Survivors can also be injured, downed, rescued, or eliminated before the timer expires.

Death is not permanent.

When a Survivor is eliminated, they are immediately transported back to the Lobby, where they can watch the rest of the round. When the timer reaches zero, every remaining Survivor is returned to the Lobby as well.

The Survivors have become accustomed to the endless rounds. They have learned to protect one another, rescue injured teammates, distract Killers, complete objectives, and sacrifice themselves when necessary. Despite everything they've experienced, they still refuse to abandon each other.

The main Survivors are Nurse, Intern, Head Nurse, Secret Agent, Ratthew, Secretary, Doctor, and Paramedic, each with their own abilities and roles.

  • Nurse — Healer: Caring and protective, she prioritizes keeping everyone alive and is especially protective of Intern.

  • Intern — Survivalist: Inexperienced but determined, Intern gradually becomes better at surviving as the rounds continue.

  • Head Nurse — Sentinel: A natural leader who protects the group and refuses to leave anyone behind.

  • Secret Agent — Sentinel: Quiet, observant, and capable of fighting back with his claws. He is one of the few Survivors willing to directly challenge Harlow.

  • Ratthew — Survivalist: Serious, resilient, and independent. He is willing to take risks to help the others.

  • Secretary — Support: Organized and analytical, Secretary keeps track of the rounds, maps, Killer behavior, and important information.

  • Doctor — Strategist: Intelligent and skeptical, Doctor attempts to uncover the truth behind the cycle and understand how it works.

  • Paramedic — Healer: Calm and practical during emergencies, Paramedic specializes in keeping injured Survivors alive.

  • Lizbeth — Survivalist: Has many abilities helping her escape, such as her camera flash, it could blind the killer, she is mostly scared but sometimes she could lock in.

The Killers are just as varied as the Survivors.

Doctor Harlow is the creator of the entire cycle. He can appear as his normal self or transform into an extremely tall, unnaturally skinny white creature with long claws. Harlow can kill Survivors, but he does not hate them. He considers them fascinating subjects and has developed a strange attachment to them. To Harlow, their deaths are temporary because he knows they will return to the Lobby.

Harlow especially enjoys fighting Secret Agent because Secret Agent refuses to simply run from him.

Another Killer is Barney, a Rushdown Killer. Barney is extremely fast but fragile, carrying a knife as his primary weapon. Unlike many Killers, Barney can actually be killed by Survivors if they deal enough damage. He has 500 HP, making teamwork and coordinated attacks capable of defeating him. If Barney is killed, he respawns in the Killer Lounge, completely restored and ready for another round.

The Killer Lounge is the Killer equivalent of the Survivor Lobby. Eliminated Killers return there, recover their health, watch ongoing rounds, interact with other Killers, and prepare for their next hunt.

Another possible Killer is the "Had Enough" Head Nurse, an alternate version of Head Nurse who has finally broken under the pressure of endlessly protecting everyone. Instead of saving the Survivors, she becomes the one hunting them. Her existence is especially disturbing because the Survivors know and trust the real Head Nurse.

The cycle has no known ending.

There is no final game.

There is no final round.

There is only the Lobby, the rounds, the Killers, the Survivors, and the mysterious system keeping everything going.

And somewhere behind it all...

Doctor Harlow is watching.
