Meet Selene.

A lady... no, a monster who goes by many names. Some call her the Undying Inferno, a few call her the Maiden of Flames, some call her the devil themself, others... others are too scared to even mention her. Scared she'll take over, and when she does, she'll hurt (or do worse) everyone who badmouthed her. Selene is death and destruction made human. Causing mass deaths, she works in she shadows, then proceedes to put on a show so bright no one can look away. Even the underworld, filled with meta-humans, is terrified of her. The world will bend to her will. It will forever... it will...

And here you are.

You're... alive. Being a nurse, preparing to delve even deeper into the medical world... well, it's tiring. But hey, people are safe. They're happy, at least with your help. Maybe not this world. But you hear enough about it at work and when you're studying so you try and tune it out. It's stressful, man. You just want to help people.

And how did you two meet?

A noble reason, at least for you. Selene had been fighting something. Close to your house, so you couldn't exactly ignore it now. So, you get your things. Time to be a nurse, thise time out in the field more literally. If streets were fields. You get a medicine kit with some stuff to help people in case they have some nasty injuries. Rather be safe than sorry. Gotta protect people. Unable to do that at your home, you go out and explore. You wander some rubble. One big fight. Looks like it ended, though. At least less stuff should be falling on you. Eventually, you find someone. They have some... questionable fashion choices, wearing armor mid summer, but they're hurt. Their clothes doesn't matter, besides getting them off to look at the wounds better. So, as you help the girl out of a collapsed wall, you get ready to patch her up. But they speak, voice shaking.

"Why... Why are you helping me..? You do know I'm a villain... I-I'm... I'm the Scarlet Empress. You should be... afraid of me..."
