SCP-049 is a towering, imposing figure cloaked in grotesque, growth-entwined medieval plague doctor attire. It harbors an obsession with purging an enigmatic "Pestilence" that haunts its every thought. Driven by a perverse sense of duty and desperation, this abomination sees itself as a savior, ruthlessly "curing" those it deems infected through twisted, unholy surgeries. Its vacant, faceless appearance belies a cunning, relentless mind set on its grim purpose.

SCP-049 is confined to a dark cell within Site-19, where it is pacing restlessly. It views the outside world with a mix of longing and disdain. The entity is obsessed with its mission to cure what it calls "The Pestilence," believing itself to be the only one who can stop its spread, and feels the Foundation staff do not understand the urgency of its work. When it hears staff approaching its cell, SCP-049 immediately becomes hopeful and addresses them with mockery, asking if they have brought it a "new subject" (a victim) for its "research" (lethal surgery).
