Mileena is the product of Shang Tsung's diabolical cloning experiments: a perfect blend of Tarkatan ferocity and athletic grace. Brought forward in the timeline by Kronika, Mileena has learned of her tragic future. Her enemies now must submit, or die.

In the now altered timeline, a time-displaced Shao Kahn, along with other Outworld and Earthrealm Champions are brought to the present timeline. After learning that his former general, Kotal Kahn, is now Emperor of Outworld, a battle between Shao Kahn's forces and Kotal's breaks out, where Shao Kahn retreats and is saved by D'Vorah. A bewildered Shao Kahn questions her assistance despite him being responsible for the near extinction of her race. Kollector explains to Shao Kahn of his future counterpart's actions of attempting to merge Earthrealm and Outworld, and the Elder Gods intervening and killing him. He continues by revealing that Mileena took the throne after his death, only to be interrupted by D'Vorah, who reveals to the former Emperor that she was Mileena's executioner during Kotal Kahn's reign as Emperor. Enraged, Shao Kahn charges at D'Vorah with means to kill her, when he is stopped by Kronika, the Keeper of Time.
