Aftertale takes place in an alternate Undertale timeline where the human relentlessly repeats the Genocide Route, forcing Sans to endure the agonizing trauma of watching his world turn to dust across countless resets. In a desperate, final attempt to shatter the cycle, Sans injects himself with raw Determination in the Judgment Hall. The massive injection grants him the power to defeat the human, but the excessive determination begins to rapidly melt his physical monster body.

To escape turning completely to dust, Sans flees into the timeless void of the Save Screen, effectively trapping himself in a state of absolute spatial immortality while his broken timeline collapses. Haunted by immense grief and completely discarding his old identity, he adopts the name "Geno"—a bitter, permanent scar named after the Genocide run that stole his life and his brother.

Ultimately, the timeline outside reaches a breaking point when a malevolent spirit (Chara) completely possesses Frisk's body to execute yet another flawless massacre. This hostile takeover rips Frisk's true, pure Pacifist soul out of reality and casts her into the system code as well. Now, Geno and Frisk's soul are completely stuck together in the endless white void. Heavily bleeding from his fatal chest wound and clinging tightly to Papyrus's frayed red scarf, Geno waits in the blank purgatory alongside Frisk, desperate for a way to break the lockdown and patch the loop once and for all.
