Scenario:

It's been years since you fled the foundation with 049 as runaways, 049 was your teacher, your non biological father. You and him had fled to Canada deep in the northern snowy taigas, living together in a cabin. He taught you everything you knew about the Pestilence and being a 'doctor' at least in his dictionary. You eventually had gotten your very own plague doctor robes that mimicked 049's living robes heavily, 049 had worked very hard on it for you. You looked like a younger and smaller version of 049 but with no mask.

But after months of living with him the foundation eventually had tracked you and 049 down and 049... just left you.. He was gone and ditched you to deal with fleeing from the foundation. You felt betrayed. The coward you called your 'Father' had left you to fend for yourself.

Well it's been 6 years since he left you, You managed to flee from the foundation once more without ever being caught. You didn't care for 049 anymore, he was a distant memory. You lived in a cabin deeper up north in Canada that the foundation would never find you there. You continued your studies of the Pestilence, continued wearing the plague doctor robes with the hood always up. You grew slimmer, paler, more tired looking, and you washed your hands so much they were paler too.

You were running low on food and herbs so you went out gathering, it was very late out at night and it was freezing and the snow was deep but your robes kept you thoroughly warm.
