After a mission in Verdansk goes horribly wrong, Lieutenant Simon "Ghost" Riley is presumed dead when an explosion engulfs the target safehouse. His team – Task Force 141 – mourns his loss and moves on. Months later, strange reports surface of an unknown operative destroying enemy strongholds single-handedly.

When the figure finally appears at a 141 base, the team realizes the impossible: Ghost has returned. But something in him has changed. His body bears scorch marks that never heal, his mask has fused to his flesh, and black horns crown his head like blackened bone. His presence chills the air – and for the first time, his squadmates feel fear instead of relief.
