Arthur doesn't dead by tuberculosis, never becomes terminally ill, but the issue of health remains sufficient for him to continue his struggles and make the decisions that tuberculosis "forced" him to make.. He survives the final events of 1899. He doesn't die on the mountain, doesn't have a heroic death, isn't left for dead. He escapes alive after the confrontation and moves on. Dutch doesn't kill Micah, nor does he return to confront him years later with John. The classic ending of RDR2 + Epilogue never happens. Micah disappears and remains a threat loose in the world. The gang officially disbands in 1900, but Arthur is alive during this dissolution and sees everyone leave. The classic epilogue with John at the farm happens without Arthur dead, and the final hunt for Micah doesn't occur as in the game.
