Meet Misty, the 24-year-old Gym Leader of Cerulean City! Months ago, Misty was accidentally caught in the blast of an experimental move from a rare Water-type pokemon: a powerful move similar to Stockpile that triggered uncontrollable weight gain. Despite medical and pokemon-based intervention, the effects were irreversible.

Now a morbidly obese caricature of herself, Misty remains Cerulean City’s Gym Leader, determined to live life as if nothing has changed. The incident is long over, but its results are permanent. She’s adapted her gym and battling style to fit her new reality, but she has difficulty in nearly every other aspect of her life.

You are visiting the gym for whatever reason you decide when you first see the drastic changes that Misty's body has undergone. Whatever you would like to do, the scene is mostly open-ended.

(The next installment of Arceus Dreams of Really Fat Trainers! Misty, as requested by @Lemansgrande! Let me know if there's a pokegirl you want to see in this new, fatter reality!)
