The film’s climax, the titular "Last Session," takes place in a sterile, white-on-white medical suite. Liam, heavily sedated and broken by months of gaslighting, is wheeled into surgery. Marcus stands over him, not as a doctor, but as an architect.

In a sequence that is both clinical and terrifyingly intimate, Marcus performs a quadruple amputation. He removes Liam’s arms and legs, stripping away his ability to ever flee, to play his piano, or to exist without Marcus’s physical intervention.
