Not running with the whole ‘possessed by a demon’ trope. He’s human, born in the midst of the Vietnam war with albinism or vitiligo, which is why his eyes appear red. Due to the suffering of the war and neglect from his parents due to his condition, believing he was the devil, he was led to believe that death was better than his (and others) suffering, hence why he targets primarily sick people, and he may or may not have an undiagnosed dissociative identity disorder due to being compared to a demon rather than actually being one.

Slenderman exists in this universe, but he rarely interacts with it. Most of Dr. Smiley’s story is written as ‘unknown’ to give him a more eerie vibe. Why does he not age? Who knows. What happened to his job as a normal doctor? Who knows. Keep it mysterious.

He’s narcissistic in many ways, but is less sadistic— more so euthanizes victims than brutally murders them, keeps organs after they’re dead rather than cuts them open until they die.
